Oakwood Medical Centre understands how important it is to keep your personal information safe and secure and we take this very seriously.

We have taken steps to make sure your personal information is looked after in the best possible way and we review this regularly.

 

How your information will be used

 

The surgery collects and uses information that you provide to undertake our duty of care and deliver services to you. Where necessary and legally allowed, in line with data protection and privacy duties, we will share your information with external organisations, commissioned partners and contracted service providers, to support the delivery of care and services to you.

 

Any information we collect may be used to assist us in improving our services to ensure you get the best possible care and treatment.

 

We may pass your personal information on to the following organisations or people if they require your information to assist them in the provision of your direct healthcare needs, some of these include;

 

  • Hospital professional
  • Other GP's or doctors
  • Pharmacists
  • Nurses and other healthcare professionals
  • Dentist
  • Any other person that  is involves in providing services related to your general healthcare including mental health professionals.

Other people who we may provide your information to are;

 

  • Commissioners
  • Clinical Commissioning groups
  • Local authorities
  • Community health services
  • Public Health
  • For the purposes of complying with the law, eg police, solicitors, insurance companies
  • Extended access
  • Data extraction by CCG
  • Anyone else you have given your consent to, to view or receive your record, or part of your record. Please note if you give another person or organisation consent to access your record, we will need to contact you to verify your consent before we release that record. It is important that you are clear and understand how much and what aspect of your records that you give consent to be disclosed.

Mean published earnings

All GP practices are required to declare mean earnings ( i.e average pay) for GPs working to deliver NHS services to patients at each practice.

 

The average pay for GPs working in the practice of Oakwood Medical Centre in the last financial year (14/15)was £35,038 before tax and national insurance.

This is for 3 full time Gps,3 part time Gps and 2 locum GPs who worked in the practice for more than six months.

 

The average pay for GPs working in the practice of Oakwood Medical Centre in the last financial year ( 15/16) was £ 58,997 before tax and national insurance.

This is for 3 full time GPs and 2 part time GPs who worked in the practice for more than six months.

 

The average pay for GPs working in the practice of Oakwood Medical Centre in the last financial year (16/17) was £38,941 before tax and National Insurance.

This is for 3 full time GPs, 3 part time GP's and 2 locum GPs who worked in the practice for more than six months.

 

The average pay for GP's working in Oakwood Medical Centre in the last financial year (17/18) was £55,392 before tax and national insurance. This is for two full time GP's and three part time GP's who worked in the practice for more than six months.

NAMED GP

All patients will have been informed of their named GP by 31/3/16.

However patients can still make an appointment with any doctor, they donot have to see their named GP .

Upgrade our telephone system

We listened to you when you asked for a place in the queue system to be added to our telephone system.

As from 4/2/15 our telephones have been upgraded,so when you are calling you know where in the queue you are situated.

This will enable patients to call back at a less busy time if happy to wait.

 

On August 23rd we repaired the paving blocks arount the larger oak tree ,on the path way leading to the practice. The tree was alos reduced to allow more light.

 

 

At Easter 2014 we repaired all the pot hole sin our private road and the large one in the carpark.The blocked drain in the carpark was also cleared.

 

All of this has been as a result of patient feedback and meeting with our PPG ( Patient Participation group)

New members welcome,next meeting Thursday 30/4/15 at 9.30am here at the surgery,come along have your say. Under 18s also welcome.

 

As requested by our PPG the paving slabs leading to our surgery have been lifted and relaid to prevent trip hazzard. New pot holes in our road were also re filled all this work was carried out on 22/10/15.

 

At the request of our PPG we have replaced our welcome sign in the carpark and addressed patient parking only .(DEcember 2015.)

We are currently addressing their request to have an additional disability parking space.

Summary Care Record (SCR)

Currently all patients personal information i.e name,address & NHS number etc are held on the patient demographic spine (PDS).

 

A summary care record will soon be up loaded on to the PDS for all NHS patients, it will contain information for health professionals to access across the country, and will contain your medication,known allergies and previous adverse reactions.

If you do not wish for this information to be uploaded onto the patient demographic spine ,then you must inform us by completing an 'opt out form' available at reception or on the summary care record website.

Once we receive this opt out form for each individual,we can then ,and only then code your records accordingly ,so that your medical information is not uploaded.

I must reiterate that your personal information will remain.

The completed opt out form will also be scanned to your records,you can choose to opt in again at a later date if you wish.

 

As part of our vision on line access soon you will be able to access a summary of your own medical record,this will be a minimum of access to allergies/medication.This is a contractual requirement by 31/3/15.We in the process now of havining this updated on our system.

Personal Medical Services (PMS) Pilot Status

We were granted PMS Pilot Status from 01/04/01. Whilst PMS provides the same type of services as General Medical Services (GMS), they are delivered under a pilot scheme that gives us flexibility and opportunity to innovate. We can offer different options for addressing local service problems and bring about improvements to both the access and quality of services provided.

 

Our pilot focuses on mental health and coronary heart disease/diabetes. We hope to more effectively meet the key aims of Barnet, Enfield & Haringey’s Health Improvement Programme as well as those of NICE and the National Service Frameworks for Mental Health, Coronary Heart Disease and Stroke.
